RULES OF PLAY FOR 50/50 RAFFLE DRAW License #RAF1329045 Purchaser must be 18 years or older and within Ontario when purchasing a ticket. Tickets will be sold using electronic raffle system supplied by Ascend Fundraising Solutions. Tickets can be purchased online through www.lwdhf5050.ca or if in person sales are available you can do so during office hours (Monday-Friday 9 a.m.-4:30 p.m. at LWDHF Offices, 21 Sylvan Street West) using a p.o.s. system supplied by Ascend Fundraising Solutions (Cash only). Tickets can be purchased for $10.00 for 20 numbers, $20.00 for 60 numbers, $40.00 for 400 numbers. After buying, patrons will be given by the sellers, a receipt with their 50/50 registered numbers from a Random Selection System. No specific numbers can be given as numbers are generated randomly. The winning ticket number will be selected by a Random Number Generation (RNG) system from all raffle numbers sold for the draw and be posted at www.lwdhf.com/lwdhf-50-50 Lake of the Woods District HospitalFoundation Staff and Board of Directors, as well as their spouses, children and children-in-law are not prohibited to win.

It is possible for the same number to win both the early bird prizes and the grand prize. One cash jackpot prize will be awarded for each draw, the winning ticket will be posted on the www.lwdhf.com website. If information about the winner is collected at the point of sale, the winner will be contacted directly. The winning ticket holder has 6 months in which to claim their prize from the date of the draw their ticket was purchase for. The winner must provide his/her name, address and phone number with picture identification for our report sheet and send it to lwdhf@lwdhf.on.ca or call 807-468-9861-2469. Once the certified winner is verified, and provides identification with photo ID, they will be given a cheque in the amount of the declared prize which is, at minimum, 50% of the total sales for that game. If the winner cannot provide photo ID, then the winning proceeds will be held until such identification can be provided. In the event that no winner comes forward, the winning stub will be kept in a safe and held for 6 months. After that time period has elapsed the prize will donated to Lake of the Woods District Hospital Foundation local charity with the approval of the Alcohol and Gaming Commission of Ontario (AGCO) For all inquiries on the conduct prizing and the procedures of the lottery please call 807-468-9861-2469.
